A medical team headed by Dr Sanjeev Khulbey CTVS Surgeon Apollo hospitals, Jubilee Hills, Hyderabad has successfully performed a complex mitral valve replacement surgery on a 40 year old patient who had been suffering from acute mitral regurgitation .
The patient was referred with severe breathlessness due to mitral regurgitation. During hospitalisation the patient arrested twice, cardioverted but was successfully revived. Later he was maintained on very high ionotropic support. Since the patient requires heamodynamic support, VA ( Veno Arterial) ECMO was immediately placed to support his cardio respiratory function.
After 48 hours of stabilisation, the medical team decided to perform MV replacement surgery while the patient was still on ECMO.
The four hr long complex surgery involved replacing the valve with an artificial one accompanied with weaning of ECMO.
The patient was extubated on the next day and was discharged after a week stay at the hospital.
The use of ECMO is very well established in treatment of end stage cardio respiratory diseases however the role of ECMO in heart failure due to valvular problem is not a well traversed path.
The use of VA ECMO in this case to stabilise the patient followed by complex Mitral valve replacement surgery marks a significant milestone in cardiac treatment as only very few reported cases of this procedure are available across the globe.
